Odometer Reading: 131,406 miles
I understand that a notarized statement of the vehicle’s actual mileage may be required at the time of cancellation. Please confirm if this is necessary.

According to the terms outlined in the Service Contract, if no claims have been filed within the first thirty (30) days from the Contract sale date, I am entitled to a full refund. Since no claims have been filed and the request is within the stipulated time frame, I expect a full refund of the premium paid.
